Does Medicare Cover Hospice Care? What Families Should Know (2026)

Short answer: yes. Medicare Part A covers hospice care for people who are terminally ill, and for most families there’s little or no cost for hospice itself. If you’re reading this for a parent or spouse, I’m sorry you’re going through it. Here’s what you need to know.

I’m Michael Cubell, a licensed independent agent with Triangle Life & Health (licensed since 2013, NPN 16899969). I help people in 15 states sort through Medicare every day. Here’s the plain-English version.

Key Points

  • ✅ Medicare covers hospice when a hospice doctor and your doctor certify a life expectancy of 6 months or less
  • ✅ You choose comfort care instead of treatment to cure the terminal illness
  • ✅ You pay nothing for hospice care, up to $5 per prescription for pain and symptom drugs, and 5% for inpatient respite care
  • ✅ Hospice is usually provided at home, including in an assisted living facility or nursing home
  • ✅ Even in a Medicare Advantage plan, Original Medicare pays for hospice. You don’t have to switch plans

Who Qualifies for Hospice Under Medicare?

To qualify, a hospice doctor and your regular doctor (if you have one) must certify that you’re terminally ill, meaning a life expectancy of 6 months or less if the illness runs its normal course. You also sign a statement choosing hospice care instead of other Medicare-covered treatments for the terminal illness and related conditions. Medicare still covers care for health problems that aren’t related to the terminal illness.

What Hospice Covers

What Hospice Costs With Medicare

ItemWhat you pay
Hospice care$0
Outpatient drugs for pain and symptom managementUp to $5 per prescription
Inpatient respite care5% of the Medicare-approved amount
Room and board at home, in a nursing home, or in assisted livingGenerally not covered, unless the hospice team arranges short-term inpatient care for pain or symptoms

That last row surprises families. Hospice covers the care, but not the cost of living in a facility. Long-term custodial care is usually paid privately, through long-term care insurance, or through Medicaid if you qualify.

Hospice With Medicare Advantage or Medigap

If you have a Medicare Advantage plan, Original Medicare is billed for your hospice care, and you don’t need to leave your plan. Your Medicare Advantage plan can still cover services that aren’t related to the terminal illness.

With a Medigap plan, the small hospice cost-sharing (the drug copays and respite coinsurance) is covered fully by most plans. Plans K and L cover part of it.

Can You Stop Hospice?

Yes. Hospice isn’t a one-way door. If your condition improves or you decide to pursue curative treatment again, you can leave hospice and go back to your regular Medicare coverage. Medicare also lets you change hospice providers. To find Medicare-approved hospice providers, use Medicare.gov Care Compare.
